As a solo RMT, I am committed to protecting your personal information in accordance with the Personal Information Protection Act (PIPA) of Alberta, Canada.

This Privacy Policy outlines how your personal information is collected, used, and safeguarded when you receive services  from Raven's Revitality Nest.

When you book a service, complete an intake form, or communicate with Raven's Revitality Nest, the following personal information may be collected:

• Home Address (for mobile appointments)

• Health History (medical conditions, allergies, medications, injuries, treatments) ​

2. Purpose of Collecting Information

Your personal and health information is collected for the following purposes:

• ​To assess your needs and provide safe, effective massage therapy services

• To comply with professional standards and legal requirements 

• To communicate appointment reminders and updates

• To maintain accurate records for insurance and billing (if applicable)

Your information is collected with your consent and is used solely for the delivery of care.

3. How information is Stored and Protected

Your information is stored securely in either locked physical files or encrypted digital systems accessible only by the therapist.

Reasonable steps are taken to prevent unauthorized access, loss, misuse, or disclosure of your information. 

 

I will never sell, rent, or share your personal information with any third parties unless required by law (e.g., in the case of subpoena, mandatory reporting obligations, or insurance audits).

 

4. Retention of Records

In accordance with Alberta regulation, client records will be retained for a minimum of 10  years following the last treatment date.

After this period, records may be securely destroyed.

You have the right to:

• Request access to your personal records

• Request corrections to inaccurate or incomplete information

• Withdraw your consent at any time (subject to legal and contractual restrictions)

• Ask for deletion of your information (where applicable)
